The mission of the Fountain Middle School Athletic Program is to create an environment that maximizes student participation, emphasizes character development, and promotes the exploration of a wide variety of athletic offerings that will contribute to the student-athletes' emotional, physical, social, and intellectual development.

Sports Physicals Event Happening at Fountain Middle School

Address: 515 N. Santa Fe. Ave. Enter through the main entrance.
Cost: $20 per student. CASH ONLY
*FFC8 students ONLY. Any student planning to participate in a sport this year needs an active physical.
Registration
The Fountain Middle School Athletics Department requires an athletic participation form as well as a physical that will remain valid through the duration of the season in which the student athlete wishes to participate. Student athletes will need to have BOTH their registration form and a current physical on file BEFORE they may participate. (These forms are valid for MIDDLE SCHOOL registration ONLY.) Registration is now being completed ELECTRONICALLY. Please use the provided links below to complete both Participation Form and upload your students Sports Physical. The school's Form, which is provided below, or your child may obtain a physical copy from Ms. Jenkins in the counseling office, the physical MUST annotate that student is cleared to participate in sports with our without restrictions.
